Continuity of care
Care that continues as needs change.
Older adults often need more than a one-off consultation. Their care may involve several medical conditions, multiple medicines, changes in mobility or memory, and support from family members or carers.
Remedine provides continuing primary care for these changing needs. We take the time to understand the patient’s medical history, current priorities and living circumstances — not only the problem that brought them in that day.

Senior care is rarely limited to one symptom. We consider medical conditions, medicines, mobility, memory, nutrition, independence and home support together. Where appropriate, we coordinate with family members, carers, hospitals, geriatricians and other specialists.

For families and carers
You do not have to coordinate everything alone.
With the patient’s consent, we can include family members and carers in consultations, explain the care plan clearly and help identify the next step. Relatives living overseas may also contact the clinic to discuss how an appointment can be arranged.

Before you arrange senior care.
- Does my parent need to attend the clinic?
- Not necessarily. Care may be arranged at the clinic, through a home visit or by telemedicine where appropriate.
- Can a family member or carer join?
- Yes, with the patient's agreement or where involvement is appropriate to capacity and best interests.
- Can you review all medicines?
- Yes. Bring prescription medicines, supplements and over-the-counter products.
- Do you perform dementia assessments?
- We can perform an initial memory and cognitive assessment and recommend specialist review where needed.
- Can you assess falls or difficulty walking?
- Yes. The doctor can review mobility, balance, medicines and other contributing factors.
- Can my parent be reviewed after hospital discharge?
- Yes. Bring the discharge summary, updated medicine list and relevant results.
